This is an excellent opportunity for a motivated practitioner with a strong background in community-based adult social care and rehabilitation.Key RequirementsMinimum 2 years post-qualified experience (PQE).Experience in reablement services, supporting individuals to maximise independence.Proven experience in reviewing and reducing moving and handling requirements, including the implementation of single-handed care practices where appropriate.Ability to work effectively within the community, managing a varied caseload and collaborating with multidisciplinary teams.You must have 2 years PQEImportant InformationThis role is exempt from the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975.
