About the role
If you’re passionate about providing high‑quality patient care, enjoy working collaboratively, and have strong experience in long‑term condition (LTC) management, we’d love to hear from you.
As a key member of our clinical team, you’ll deliver a wide range of nursing services and support patients in managing chronic health conditions. We are particularly interested in someone who can take a proactive, holistic approach to patient care.
Your responsibilities will include:
- Management and review of patients with long‑term conditions (e.g., diabetes, asthma, COPD, hypertension)
- Supporting long‑term condition clinics and QOF-related work
- Providing high-quality general practice nursing services such as immunisations, wound care, cervical screening, health checks and lifestyle advice
- Working autonomously while contributing to the wider multidisciplinary team
- Supporting practice initiatives to improve patient outcomes and access
