About the role
As an Occupational Therapist with Sussex Community NHS Foundation Trust, working 36 hours a week on a three-month contract, you will provide specialist assessment and intervention to support individuals in housing and community settings. Collaborating with service users, families, carers and multidisciplinary teams, you will deliver tailored programmes that promote independence, safety and wellbeing within their home environments.

Conduct thorough assessments of functional abilities and housing-related needs.

Develop, implement and review personalised intervention plans to support independent living.

Work closely with service users, carers and professionals to coordinate care and access to community resources.

Maintain accurate and up-to-date clinical records in line with NHS and professional standards.

Provide advice, education and training on adaptive equipment and environmental modifications.

Contribute to service development and improvement initiatives within housing and community occupational therapy services.
