Adult Social Worker – Hospital Discharge
West/Central Cornwall
£32.50 per hour
37 hours per week
Hybrid working – minimum 2 days per week face-to-face

We are currently recruiting for an experienced Adult Social Worker to join a busy and supportive team covering West and Central Cornwall.
The role focuses on supporting hospital discharge and completing assessments for adults returning to the community or moving into Discharge to Assess placements.

Key responsibilities:

• Complete Care Act assessments
• Undertake Mental Capacity Assessments and Best Interests decisions
• Support safe and effective hospital discharge
• Follow up assessments within Discharge to Assess placements
• Complete face-to-face assessments in hospitals, care homes and people’s own homes
• Work collaboratively with health colleagues and other professionals
• Manage a varied caseload and contribute to positive outcomes for adults
• Work with cases involving the Court of Protection where required

Requirements:

• Social Work England registration
• Qualified Social Worker with experience working with adults
• Hospital discharge experience is highly desirable
• Strong knowledge of the Care Act and Mental Capacity Act
• Experience of Best Interests decision-making
• Court of Protection experience desirable
• Must have access to a car and be able to travel across Cornwall
• Remote-only candidates cannot be considered
