Responsibilities:

• Conduct prompt, person-centered assessments for adults referred to the Duty Team.
• Work collaboratively with multi-disciplinary teams, including health professionals, housing services, and voluntary organisations.
• Develop and implement care and support plans tailored to individual needs.
• Manage crisis interventions, ensuring the safety and well-being of service users.
• Safeguard adults at risk by identifying and addressing concerns, adhering to national and local safeguarding protocols.
• Maintain accurate, up-to-date case records and reports in compliance with legal and organisational standards.
• Provide advice and guidance to individuals, families, and carers.
• Contribute to service development and promote best practices within the team

Qualifications and registrations:

• Qualified Social Worker
• SSSC registered in the Social Work part of the register

Skills:

• Car driver
