About the Role

As an Assistant Care Manager your duties will include:

  • Working in a team taking referrals, triaging, and identifying eligibility for Adult Social Care
  • Provide information, advice and guidance for referrals that need signposting to other partners and universal services.
  • Undertaking strengths-based, person-centered and outcome-focused Care Act needs assessments, support planning and proportionate short-term interventions. To identify risk and where appropriate to the role and competence, seeking support where complexity requires and raising with the senior practitioner.
  • Effective liaison with the wider team and other Independent and Voluntary Agencies
  • Updating and maintaining case file notes and electronic records
  • To work in an integrated way with partners including health and colleagues across LLR and locally


Additional Requirements

The successful candidate will be required to undertake an enhanced Disclosure and Barring Service (DBS) check. Costs for the DBS are covered by Rutland County Council.

Please note that you will need a full driving licence to enable you to travel across the county.


About You

We are looking for someone who is committed to delivering high-quality, person-centred support that promotes independence, wellbeing, and positive outcomes for adults with care and support needs.

We would expect the individual to have a good understanding of adult social care legislation, safeguarding responsibilities, care assessment and support planning processes, and partnership working.

Strong communication, and organisational skills are essential, along with the ability to build effective relationships with service users, carers, health professionals, and partner agencies.

The successful candidate should be capable of managing competing priorities, maintaining accurate records, and contributing to service improvement while ensuring compliance with statutory duties, policies, and quality standards. A proactive, compassionate, and solution-focused approach, combined with a commitment to equality, diversity, and safeguarding, would be highly valued.
